Tests for incremental tablign with trie-indexed dynamic code and tests for incremental tabling using abolish_all_tables and abolish_table_call.

Added code to abolish_table_calls to reinitialize hash tables for
incremental tries after all the hashtables have been destroyed.

This fixes things for incremental tries, but I think we still have the
same problem of destroyed hash tables for regular dynamic code.

Reduce large int to be smaller than max-int to avoid error message.

This commit was manufactured by cvs2svn to create branch
'latest_plus_supp_branch'.

Improved error messages for low-level ptoc_ type functions (and a few others)
Now print the actual term that is found that is incorrect. May help localize
errors better.

Changed xsb_query_string to use atoms (and not code-lists.) It is quite a bit
more efficient, and we now have atom gc, and with Michael's query caching, the atoms
are created anyway.

Added space for final null character for constructed file names. %$#$%#!

Changed calls to findall/4 to bfindall/4 so that debugger works with setof.

switched to fast_trie_interned

Returning leaf in fast_trie_interned.
