bug fix in flora_decde_predicate

Fix shave typo.

(1) enable bform in rule heads
(2) enhance flrwrapper. new type 'flora' added that can be
used to decoded builtin predicates in trailers
(3) encode predicates in the trailers like firstorder but
with the normal prefix (not first order prefix)
(4) add new directive for .fli files:
FL_FLOGICSTATIC for firstorder like trailer predicates
FL_FLORAPATCH for generating Flora-specific patch rules
(5) enhance loading & compiling routines

Some changes to reflect recent code additions, get_char/2, put_char/2 etc.

moved the debugger to a separate dir

Forgot to checkin the .O file for previous fix to indexing.

(1) different prefix for (first order) user module predicate,
system module predicate. (look at flora_prefix.flh and flrheader.flh)

(2) incorporate @flora(mod) (parsing, compiling, loading)

(3) new interface in flrwrapper.P

Minor update to describe interfaces that do and dont need to be configured,
as we always get a lot of questions on this.

Brought intro a little more up to date, mentioning constraints, interprolog, and flora.
