#142 repeats within case

This is with XSmiles 0.92 (Linux and Windows). Repeat
constructions within cases do not get populated. It
works with XSmiles 0.82 (Windows anyway).
If you remove all the switch/case elements from the
document attached, then all the controls get populated,
otherwise you get a:
ERROR: XForms fatal error: component was null, the
component was never created.
