Charles, It looks like all of that setup work is taking place within the main method of com.tivo.hme.host.sample.Main and then completing at 2008/07/13 21:43:29. Java things it is supposed to quit as soon as your main method completes which makes my think that there are not any daemon threads being started as part of your application. There is no way for me to tell on this end what would be different depending on how you launch your application. I don't see any sign that anything is being done incorrectly with the wrapper configuration though. At the very end of your main method, try calling WrapperManager.requestThreadDump(). This will cause a dump of all threads at the same point in your application. Compare the cases that work and don't. My guess is that either your main method never completes, or that there is at least one more daemon thread running when it is working correctly.
