After a significant amount of troubleshooting a 500 line csv that recently stopped working, I have discovered that entries with a dash (—), instead of Hyphens (--), result in a csv not loading.
This is especially awesome and common if you're converting an Excel (xls, xlsx), where Microsoft has implemented an auto-complete "feature" to change double hyphens to dashes.
