Are you using PAM for authentication there? Usermin will use it if you ha=
ve the Authen::PAM perl module installed, and an /etc/pam.d/usermin file.=
 In this case, the login attempt and reason for failure will be written t=
o /var/log/authlog or /var/log/secure, depending on your OS.

and nothing in miniserv.error that seems to relate.
