Are you using LDAP or SQL for the user accounts?  I normally develop
against LDAP, with a test against SQL before checking in.

At one point this was by design, but I thought I fixed it.  Which version of
Asterisk and are you using app_cbmysql?

Did you define an outgoing CHAN_TYPE and friends in lib/defines.php ?
