Do you see that program name in the scripts table in the database?
If so maybe the authorization level isn’t correctly set.
If it isn’t there then you need something like
INSERT INTO `scripts` ( `script` , `pagesecurity` , `description` )
VALUES (z_importchartofaccounts.php', '15', 'Import Chart Of Accounts');
