Two independent volumes, same password and for both the error about a wrong password - sounds to me like you are simply entering the wrong password. Have you tried to enter your password into a simple text editor and copy and paste it into the password box? Just make sure there are no space keys at the start or the end of the password.

It really does seem to defy all reason, but this all happened after Windows ran chdsk, found corruption and successfully repaired. I've entered the same PW every day for years, tried it countless times over the past 2 days, and now tried your copy/pasting from text document suggestion.
I was hoping there's some obscure mechanics of VC I hadn't considered, but it's starting to look like magical dark sorcery is the only explanation. :-/

I could mount the outer volume without issue; only the inner volume no longer worked. At this point I've already re-formatted the whole volume, but left the USB stick backup volume in tact on the off chance it could still be accessed somehow.

Can you mount the outer volume in read-only mode to see if the primary header still exists and works?
If no, try mounting the outer volume using the optional embedded backup header.
