What steps will reproduce the problem?
1. Load USB Loader GX from the Homebrew Channel
2. Select a Game
3. Select Start

- or -

1. Load USB Loader GX from the Homebrew Channel
2. Click the Wii menu Icon
3. Select "Wii Menu"

What is the expected output? What do you see instead?
Expected output: The game loads, or the system returns to the Wii Menu, depending on the steps taken above.

Actual output: The controller immediately disconnects and the screen turns black. The controller will not reconnect and the system does not respond to the power button. The only recourse is to disconnect the power from the console.

What revision of USB Loader GX are you using?
Rev1233 IOS249 (Rev 21010) + AHB
(d2x v10beta53-alt-vWii base56)

On what operating system (IOS/Firmware/System)?
Wii U version 5.3.2 U
SysCheck output: http://pastebin.com/tX3Q71AP

Please provide any additional information below.
I'm using a USB drive formated to FAT32 and .wbfs files. I'm running vWii inside of the Wii U. I was able to play previously every game on the drive with USB Loader GX before this behavior occurred. No games were added and no settings were changed from the time it was working till now.

I have not tested yet with any actual Wii games on disc.

After a bit of troubleshooting I was able to resolve this issue. The things I did:
    1. Delete all Wii save data.
    2. Power off the Wii U and disconnect everything for ~ 5 minutes
    3. Reformat the USB Drive and reload the games onto it.

I'm not sure which step fixed the issue but I suspect it was #3. Hopefully this will help anyone in the future that runs into a similar problem. Please close this issue.

that's strange that it happened without changing anything.
You should have tried step by step to find what was wrong, starting by reseting the loader's settings.
then installing the loader again (or updating to 1239).
    then try another device/HDD.
reformat should be the last thing to try (unless you don't care about it, but some users have hundreds of games and data on it and it's long to put everything back on it).

Sorry to bump this but I'm still encountering this issue pretty consistently. I've found that I've been able to load any game 0~5 times before all games stop loading. Reformatting the USB Drive doesn't seem completely necessary as any read/write operation to the drive is enough to resolve the issue.

Recently I've also encountered an issue where occasionally simply launching or exiting USB Loader GX will result in an unresponsive black screen.

None of the loader's settings have been changed since it was installed and it's been updated to Rev1239

Deleting+formating+reinstalling everything don't tell me where the problem is located.

when you test something, do it step by step.
    1. reboot the console (you said you have to force shutdown, so it's rebooting. After full shutdown it still happens 100% of the time?)
    2. if still happening reset the setting
    3. If still happening : If you have the loader and setting on SD card, be sure the card is not write protected as it needs to write the settings when exiting the loader.
    4. if still happening try another USB device instead of formating the drive. If it works then come back to the problematic drive and see if it happens again with this drive. If it happens then it's the drive not the loader. try to see what's wrong with your drive. (I don't think it's a drive issue, it doesn't try to access it when exiting unless you have the boot.dol on USB instead of SD card)
    5. Try older version and provide the first revision which trigger this problem.
link to all revision is on the homepage.
