    You probably are not using d2x as default IOS (verify that you don't boot using Hermes cIOS), or are using a wrong base, or are launching the loader from Priiloader instead of using the forwarder.dol, or are launching from and old HBC, or using new HBC without <no_ios_reload/> or <ahb_access/> tag in the meta.xml.

    If you don't use an SD card, then you put the boot.dol on USB?

    Then you need:

    usb:/apps/usbloader_gx/boot.dol
    usb:/apps/usbloader_gx/icon.png
    usb:/apps/usbloader_gx/meta.xml

    The meta.xml is what grants you NAND access if you launch from Homebrew Channel.
    Open the meta.xml in a text editor (notepad) and verify that you have the line:
    <no_ios_reload/>

    If you launch from the system menu with a forwarder channel you created:
    You need a Channel with AHBPROT (download and install Forwarder Channel v5.1)
    https://code.google.com/p/usbloader-gui/downloads/detail?name=USB%20Loader%20GX-UNEO_Forwarder_5_1_AHBPROT.wad
    See if it fix your problem.

    If you want to use your own forwarder, you need to remove AHB PROTection (to get NAND access. There's a bit you need to set in the channel's tmd to enable AHB access) and don't reload IOS before launching the loader's boot.dol

    Ah, and about the IOS, CFG loader says it's using an IOS, but it doesn't mean it's in the slot249.
    Check your IOS slots, reinstall d2x IOS v10(beta52) based on IOS56 in slot 249.

    You can tell USBLoaderGX to use another slot if you already have the cIOS in another slot and don't want to install it again in slot249.
    Edit the GXGlobal.cfg in a text editor and replace "249" with the slot where you have d2x base56 installed.
