#552 Need mechanism for promoting witness-level annotations to edition level

I've imported an HTML document with styled text as a witness. By the time it gets to the publication component, those annotations are no longer exposed.

One possibility is to give special treatment to outlines which have only a single witness, and promote all their annotations to the edition level.

A tidier way might be to add a mechanism to promote a verbatim witness straight through to the edition. It would automatically create a single structural annotation covering the entirety of the witness, and an outline which uses that structural annotation as it's single source. Also, it would automatically copy all the witness' annotations into the edition with the proper anchors to the edition text.
