For macros returning a Genshi Element or Stream, using `to_unicode()` would lead to XML serialization. Replace this by using the `_markup_to_unicode`, which uses XHTML serialization as needed.

Make a few more comments internal ones. Hints about the ticket structure make sense in the final output, but not comments about sections that won't be present in the output.

0.12dev: In ticket queries, changed "contains" matching for the `keywords` field to match all specified keywords. Additionally, keywords can be excluded by prepending them with a "-".
