Fix some minor style glitches

* Applied patch from mrowe that fixes the revision log when multiple files are copied/renamed in the same changeset. Closes #514.
* Fixed a typo in Ticket.py that did break ticket description preview.

Move tabindex of wiki toolbar far away from the normal range of controls

Bold before italic button in the wiki formatting toolbar. Use more familiar naming.

Fix hint for horizontal rule in wiki formatting toolbar. Thanks vittorio\!

Rework of the wiki toolbar:
* The toolbar is built entirely from trac.js, resulting in less javascript code in every generated file that includes the toolbar
* Use standard DOM methods instead of document.write() for generating the toolbar
* Only show the toolbar for IE and Gecko-based browsers, the only ones that currently actually support the functionality
* Use standard HTML tooltips (title) instead of a text input field for displaying command info
* Combine all toolbar images into a single image file

r123 instead of v123 column headers in inline-diffs for changesets. Also, provide non-abbreviated string as title.

Fixes bug in shorten_line. No more too-short messages in the timeline.

* Automatically submit change from/to inline/side-by-side diff for Wiki pages
* Long, non-wrappable lines no longer run out of their containing box in side-by-side diffs
