Another attempt at fixing #6211 by clarifying the `IPermissionPolicy` API doc a bit more and providing a sample plugin.

Another issue with the TracRevisionLog: setting the `stop_rev` to a revision ''newer'' than the last revision on that path was wrongly triggering a "non existent path" error.

Fixes #6381.
