Get rid of the <!DOCTYPE> in the macros.html.

This is from where the <!DOCTYPE> line was coming from in generated fragments.
Also, now that it is gone, we don't need to remove it manually in the expand_dir.js Javascript end,
before retrieving the directory entries.

context-refactoring: rename get_* to *_of in the templates (`url_of(resource)` etc.)
