FieldRefactoring: refactor the rendering of a Milestone link

Triage: Direct the Update button to the correct place.

Triage: Code from old version that I forgot to change.

Triage: Add the changed-value watching and styling.

Triage: Starting the JS work, adding a new permission TICKET_TRIAGE, and integrating this with the ctxtnav.

FieldRefactoring: new ticket and ticket change can share more code for the initialization of the fields sequence.

Ticket preview mode: show the ''ticket can't be saved'' notice outside of the ticket change preview, as the latter is not always there (e.g. for new tickets) and it's anyway clearer that way.

Follow-up to r5449, ticket._old is is not empty for new tickets. The wrong check prevented adding tickets for anonymous users. Thanks to Jim Arnell for pointing out the issue.
