Try to persist changes in the session ''after'' the request has been successfully processed, but ''before'' the content is actually sent. That way, errors like database locks that are triggered by this won't go unnoticed.

Fixes #3503.

setuptools branch:
* Remove all traces of the `default_dir` function
* `$prefix/plugins` is now configured using the `plugins_dir` option in `[trac]` in `trac.ini`
* Simplified the plugin loader
* Removed support for old-style wiki macros, as supporting those is hopeless, and they relied on `default_dir`.
