Commit [r3353]  Maximize  Restore  History

Fix the `ImageMacro` doc w.r.t style arguments (fixes #3184)

Also cleaned-up the way the !ImageMacro formed some of the hrefs.
This in turn resulted in some cleanups in the Formatter
and in its associated tests.
