Some fixes and cleanups related to non-self contained repositories (#1830)

We only use the ''created path'' of some node if that created path stays
within the scope of the repository.

Also, when generating changeset node change information,
we check whether the base path is within the scope or not,
and if we have the access right to it.
In the negative, the base path is set to None,
which means that edits or copies might then appear as added files.

Ported [3172] to 0.9-stable ''(wiki_to_* cope with empty wikitext)''

`wiki_to_*` functions cope with empty/None wiki text. This removes the need to check on every call. Closes #3044.

TracLogging improvements:
* show full date information for the file type, even for the DEBUG or ALL modes
* show time information for the stderr type

Should fix #2763.

spam-filter: add IP blacklist filter

Follow-up to [3167]: removed duplicate declaration of `database` config option from `trac.env.Environment`.

Moved the declaration of some config options to where they are actually being used.
