Reworked the way the database connections are used. Now a single
connection is passed around to all parts of the cgi. Also a general code
cleanup. I hope I didn't break anything.

* Converted the permission cache from a global dictionary to a class.
* Make sure the user has {{{WIKI_VIEW}}} permission before displaying any wiki page.
* {{{WIKI_ADMIN}}} now work as a meta permission containing every other permission. This closes #86.

New behavour when linking to subversion files.

svn:/path/file will display the file log.
svn:/path/file#rev will display the file contents for that revision.

Support wiki-links to subversion directories as well as files. This closes #85.
