The problem is that hte mime type .ica is not associated with the ica
client. THere was talk of including hat in either the ica client package
or firefox (and it started a lot of discussion re: cross-linking of
packages). Not sure the current status.

If you build locally, you can change this yourself (but you'll have to
search through the archives as I can't remember how OTTOMH).

You can set the startpage with SESSION_n_FIREFOX_OPTION=<url>
THere was also some discussion on this at least 6 months ago (and a
couple of different options given, just can't remember right now...:-)
