Then again TSynEditPlugin is your friend. You will need to override the 
paint method of it to do that.

But  looking at your problem in a different way, maybe you should design 
this way:
Say your target language is pascal. If someone wants to ignore some part 
of the code, you just wrap out that code with  "{IGNORED" and  "}".  And 
then to highlight this text differently, you can simply modify the 
default pascal highlighter(or the hl. of your target language). Also 
this way you dont have worry to about line inserts or deletes regarding 
ignored parts.
