The "Create new client" form is not showing correctly in Internet Explorer version 7. See attached file for screen shot.

To recreate this bug (in IE version 7)
1. Login to the system using a user account
2. Click on the "Create a new client" link from the menu on the left.

The form looks fine on FireFox. I don't know how it looks in IE version 6.
