Removed old diagnostic code, simplified comparison methods, added

Added move methods to allow for trees to be combined without copying,
renamed append->insert for consistency, leaving append methods as
undocumented backward-compatibility methods

C++0x mode doesn't like me passing 0 as the value of a void* to a
template function
