Reworded explanation to clarify what the problems are in the STL and
how SLplus iterators solve them.

Removed old diagnostic code, simplified comparison methods, added
diagnostic messages to exceptions

Added move methods to allow for trees to be combined without copying,
renamed append->insert for consistency, leaving append methods as
undocumented backward-compatibility methods

C++0x mode doesn't like me passing 0 as the value of a void* to a
template function

DJDM's addition - operator< to allow triple/foursome to be used in a map

Added stlplus3.hpp header file for monolithic build only
