Fixed problems with compiling with Visual Studio 8

Regenerated Visual Studio Project files using the "make vcproject" command

Switched back to using smart_ptr to manage the substructure

Put whole file into namespace stlplus rather than individual elements

Put whole file in namespace stlplus rather than individual bits. Install CLI
error messages in message handler if not there already - so there's no need to
have stlplus_messages.txt. Some tidying up.

Added stlplus:: prefix to constants so they work without a using clause. Also
made data elements private and used smart_ptr_nocopy to make a cli parser uncopyable

Fixed indent and removed unneeded include

Use dprintf instead of string streams for formatting - also change string to
char* to simplify interface
