What steps will reproduce the problem?
1. Install latest version (1.5.1)
2. Restart

What is the expected output? What do you see instead?
Expected to be able to right click toolbar/menu areas of explorer and
enable "StExBar".  Accessing the same functionality via the menubar,
View->Toolbar->StExBar shows the same result.

What version of the product are you using? On what operating system?
1.5.1 on Windows XP SP3, latest patched/updates as of today

Please provide any additional information below.
I see some people use Process Explorer, DLL watchers, etc. to help debug,
let me know how I can help.

    Try enabling/disabling your other toolbars - one of them might enable the StExBar
    instead.

    Unfortunately, I have no idea what is causing this - the StExBar registers correctly
    in the registry (at least as far as I can tell - it registers according to how the
    docs suggest).

    reformat/reinstall of Windows, this is back up and working now.  Similar environment
    compared to pre-format state, but I'll keep an eye to see if it becomes disabled

    installed IE8 or a recent windows update. You need to start IE and enable the toolbar
    there because it will give you a dialog box to enable it due to the default security
    features. Once enabled in IE I was able to enable it in an explorer window.
