What steps will reproduce the problem?
1. Install latest version (1.5.1)
2. Restart
What is the expected output? What do you see instead?
Expected to be able to right click toolbar/menu areas of explorer and
enable "StExBar". Accessing the same functionality via the menubar,
View->Toolbar->StExBar shows the same result.
What version of the product are you using? On what operating system?
1.5.1 on Windows XP SP3, latest patched/updates as of today
Please provide any additional information below.
I see some people use Process Explorer, DLL watchers, etc. to help debug,
let me know how I can help.

Interesting observation:
I can enable the toolbar on the taskbar, and see all the buttons, and interact with
it - e.g. clicking on options brings up options dialog; in short seems to be fully
functionining.
Still stays greyed out in explorer->toolbars list. Also it is the only greyed out
option, i.e. all the other toolbars (stock) are able to be enabled/disabled.

I've seen it happen that the context menu entries were wrong: in my case, I had to
enable another toolbar (another name in the menu) for the StExBar to get enabled. The
menu entry names didn't match the toolbars anymore for some reason.
Try enabling/disabling your other toolbars - one of them might enable the StExBar
instead.
Unfortunately, I have no idea what is causing this - the StExBar registers correctly
in the registry (at least as far as I can tell - it registers according to how the
docs suggest).

Tried what you suggested, no resolution though.

maybe uninstalling StExBar, killing all explorer instances (or reboot), then
installing it again might help?

First: uninstall StExBar again. Then, try using the registry editor (regedt32.exe):
search for string "stexbar" and remove all keys/values you find, also remove all
keys/values of string "367D8B32-F9FD-474b-8E65-9E521F35DE99".
After that, reboot. Then install the StExBar again.
If that doesn't work, I don't know how to investigate this further.

Not quite sure what the conflict was, but since other issues required a
reformat/reinstall of Windows, this is back up and working now. Similar environment
compared to pre-format state, but I'll keep an eye to see if it becomes disabled
again. Thanks.

I had the same problem and got it working again. This seemed to occur when I
installed IE8 or a recent windows update. You need to start IE and enable the toolbar
there because it will give you a dialog box to enable it due to the default security
features. Once enabled in IE I was able to enable it in an explorer window.
