Hello, Can't help you with #2 and #3 but I do know how to fix #1: The address book page contains a header that is included in most, if not all, the files in the src dir. This header includes a JavaScript function that looks in the first form of the page for a form element. When you have no addresses in your address book an empty form is created with no elements (ie no textboxes, checkboxes, ect..). To stop the empty form from being created you will need to add two lines in your /path/to/squirrelmail/src/addressbook.php file.
