I've checked in the latest version of the DTD.

I attach an XSLT stylesheet that converts from old to new format, and an Ant
script that uses it across a tree. (The DTD isn't definitive yet, just my
local version.)

The XSLT loses comments and formatting--it was a pretty quick effort.
