SimThyr has been updated to version 4.0.6. This release fixes a bug, where certain loads (TRH tests and TBG rise in pregnancy or on oral anticonception) weren't correctly handled. SimThyr 4.0.3, a bug fix release, has been released. Thanks to Henrik Glensbo from Denmark some stability issues could be addressed. Additionally, this is the first version to be notarised by Apple, thereby facilitating easy installation on newer versions of macOS. Although it doesn't come with new features, it implements, however, multiple bug fixes (thanks especially to Henrik Glensbo for reporting several issues). Additionally, it is the first version to support the Cocoa API, 64-bit processors and the dark mode on macOS. This is a recommended update. With immediate effect, SimThyr is available in version 4.0.1. This is a bug-fix release that is recommended for all users. Fixed bugs include missing units of measurement in comboboxes in the preferences dialog box, a problem with the code signature for macOS and an occasionally arising (and nearly intractable) bug that prevented drawing of nullcline plots in certain situations. We are pleased to announce SimThyr 4.0, a major re-write of the popular virtual thyroid homeostasis software. This version features a plethora of new functions, including but not limited to null isocline plots, enhanced data export options and model annotations supporting the creation of MIRIAM-compliant models. This functionality required two years of development and a partial re-implementation of the simulation engine and more than 130 SVN commits. In additions, multiple bugs have been fixed. This bug fix release resolves some GUI-related issues with Windows XP and Mac OS X 10.10 Yosemite, and the speed of the simulator thread has been optimized. On the occasion of the World Thyroid Day we have released SimThyr 3.3. The new version introduces significant new features, first of all support for two-way sensitivity analysis. This technique allows for studying the combined effect of two structural parameters on a behavioural parameter (i.e. hormone level). Additionally, SimThyr 3.3 adopts version handling for scenario files, an option to switch off the circadian TRH rhythm and more flexible handling of units of measurement based on PUMA Repository, Last but not least some bugs have been fixed. We are glad to announce that SimThyr has been released in version 3.2.3 today. This is a bug-fix release that addresses numerous issues including the tracker items #12 to #17. Additionally, SimThyr 3.2.3 is the first version that has been code-signed for Mac OS X. This makes it easier to install the application on newer versions of Mac OS X, including Lion, Mountain Lion and Mavericks. Recently, a new paper using SPINA has been published in the Journal of Clinical Pathology. By combining in-silico modelling using SimThyr with an observation study in a large clinical sample (n=6641) it could be demonstrated that TSH levels are not unambiguously coupled to FT4 levels. To add even more complexity, GT values in different ranges were associated with a different influence of structure parameters on TSH levels in sensitivity analysis. These observations question the log-linear standard model of thyroid homeostasis and suggest that a hierarchically structured model of the hypothalamic pituitary thyroid interaction may be a more appropriate description of pituitary thyrotroph physiology. The results of this study are of considerable importance for diagnostic evaluation of thyroid homeostasis, since they demonstrate that a pure TSH-centric approach may be inadequate for diagnosis of thyroid function in some, if not most, affected patients. Abstract: Aims: Understanding the exact relationship between serum thyrotropin/ thyroid stimulating hormone (TSH) and free thyroxine (FT4) is a prerequisite for improving diagnostic reliability and clinical decision making. Methods: We (1) retrospectively studied the relationship between TSH and FT4 in a large unselected clinical sample (n=6641) of primary hypothyroid, euthyroid and hyperthyroid subjects, and (2) applied a mathematical model of thyroid hormone feedback control to assess the relation between structural parameters and TSH levels in the different functional states. Results: When separately analysing total sample and untreated subjects, the correlation slope for logTSH versus FT4 for hypothyroid subjects was significantly different from that of the euthyroid panel and hyperthyroid subjects (the latter being compromised by reaching the TSH assay's lower detection limit). As trends between functional states changed, each functional segment appeared to become differently regulated. Theoretical modelling and sensitivity analysis revealed that the influence of various structural parameters on TSH levels also depends on the overall function of the feedback loop. Conclusions: Our data suggest that the states of hypothyroidism, euthyroidism and hyperthyroidism can be regarded as differently regulated entities. The apparent complexity could be replicated by mathematical modelling suggesting a hierarchical type of feedback regulation involving patterns of operative mechanisms unique to each condition. For clinical purposes and assay evaluation, neither the standard model relating logTSH with FT4, nor an alternative model based on non-competitive inhibition can be reliably represented by a single correlation comparing all samples for both hormones in one all- inclusive group. A paper about medical decision making in patients receiving substitution therapy with levothyroxine was published in the European Journal of Endocrinology (2013: 168: 271-80). The paper that made use of the MiMe-NoCoDI model of pituitary-thyroid feedback control demonstrated that a pure TSH-centered approach may be insufficient for assessment of thyroid state in certain forms of heterostasis. Sensitivity analysis with SimThyr 3.2 showed a strong influence of various structural parameters on TSH secretion. Abstract: Objective. In recognition of its primary role in pituitary-thyroid feedback, thyrotropin (TSH) determination has become a key parameter for clinical decision-making. The present study evaluates the value of TSH as a measure of thyroid hormone homeostasis under T4 therapy. Design, Methods. We have examined the interrelationships between free triiodothyronine (FT3), free thyroxine (FT4) and pituitary TSH by means of 1) a retrospective analysis of a large clinical sample comprising 1994 patients either untreated or on varying doses of L-T4 and 2) independent mathematical simulation applying a model of thyroid homeostasis, together with a sensitivity analysis. Results. Over a euthyroid to mildly hyperthyroid functional range, we found markedly different correlation slopes of log TSH versus FT3 and FT4 between untreated patients and L-T4 groups. Total deiodinase activity (GD) was positively correlated with TSH in untreated subjects. However, GD was significantly altered and the correlation lost under increasing L-T4 doses. 95% confidence intervals for FT3 and FT4, when assessed in defined TSH concentration bands, differed significantly for L-T4-treated, compared to untreated patients. Higher doses were often needed to restore FT3 levels within its reference range. Sensitivity analysis revealed the influence of various structural parameters on pituitary TSH secretion including a preeminent role of pituitary deiodinase type 2. Conclusion. The data reveal disjoints between FT4-TSH feedback and T3 production that persist even when sufficient T4 apparently restores euthyroidism. T4 treatment displays a compensatory adaptation, but does not completely re-enact normal euthyroid physiology. This invites a study of the clinical consequences of this disparity.
