I've just looked this up.  A user reported failure to 
retrieve any reads from an indexed .bam file which had a 
single reference sequence of length 605 Mb.  A different 
.bam file with a reference sequence of length 305 Mb 
worked fine.  This was discussed in emails to the 
samtools-help mailing list between myself, Rebecca 
Sun and James Robinson, dated Monday and Tuesday, 
August 1 and 2, 2011.
