#1 Revised XML Metadata spec

The existing document did not address how to handle
metadata when more than one schema was needed, for
example, a query hit that that included both audio and
video results. Also, the existing document did not include
the "index" attribute, needed to map metadata to result
entries. This patch takes care of that. It is a replacement
for <http://rfc-gnutella.sourceforge.net/developer/testing/
metadata.htm>.
