This seems similar to ticket #23(?).

RIS 1.1.0, Win7 64-bit.

Cooper is announced to have been killed in a training accident, and for that season is shown as being both in the cemetery and assigned to a Mercury crew. He is removed from the flight crew at the start of the next turn without receiving a message that he had been killed in the 'astronauts in the news' segment of the newscast.
