Summary: Add FSRef based storage object

Initial Comment:
We should add Mac OS TQ3StorageObjects based on FSRefs rather 
than FSSpecs, to support both types of file specifiers.

The API should mirror the Q3FSSpecStorage_xxx calls, and 
Q3MacintoshStorage_GetType should be extended to return a 
kQ3MacintoshStorageTypeFSRef constant.
