On the scaling feature: This runs into problems if you have a mix of landscape, horizontal, and/or square images. The scaling is different for each kind of orientation. Using the 'Constrain to Square' setting on the Layout panel addresses this mixed-orientation problem. However, if the images are not all of the same dimensions, they will not all scale identically. If they all have the same dimensions (regardless of orientation) 'Constrain to Square' should give you what you want except for the auto-filling of rows. That is something I may be able to add in at some point in the future.

For the subheadings idea, I have something that kind of does what you want. There is a new @Metadata layer type that is going into the next release (it should be working in the current beta). If you create a text layer with the name '@Metadata=%p', that layers contents will be set to the name of the folder containing the first image on that page. If you have 'Group by Folders' set on the Source panel, you will get the name of the folder at the top of each sheet of images in that folder. When a folder is finished, the sheet is finished. A new sheet is create to start on the next folder. Embedding the subheadings in the sheet would take quite a bit of work.
