Fixed inappropriate ghosting of zoom menu items.  Required switching to using
the entry name as argument to entryconfigure instead of numerical index.  This
caused problems going from Tcl 7.x -> 8.x (I think in 8.x the tear off entry
became numbered where previously it wasn't).  I don't think this form of
reference was even available in 7.x.

Highlights include:
(1) there is finer-grained dependency checking.
(2) targets to create src (all files under cvs control) and html (all *.html
and *.jpeg files required to browse the documentation) tarballs.
(3) install copies these src and html tarballs, *.dvi, and *.ps.gz files to
the install directory as well as the unpacked contents of the html tarball to
allow browsing.
