Deal with AT conversion of bindings/tk: build and install plserver and
install *.tcl, *.itk, and palette (*.pal) files.  The examples/tcl/tclIndex
stuff is bogus; that file is generated and should (eventually) be removed
from cvs after the merge of AT into MAIN.
