Log Message:
First pass at incorporating dev.tex into the advanced chapter.  This chapter
now builds without errors so this is a good stopping point for recording all
the changes into cvs.  But a second editing pass will be required to replace
all of the remaining latexism's (i.e., tables, lists, and some
cross-references) with the DocBook equivalent.
