Split plD_bop_plm into smaller functions and rewrote substantially.
Fixed several bugs that affected familying.  Added lots of debug
output.  Changed plD_bop_plm to issue a BOP0 rather than a BOP if
it is the first BOP in a file (this allows plrender to do reverse
seeks across file boundaries in a metafile family).
