This looks familiar, a Xserver that supports both pseudo and true color visuals (not usual under XFree86, but most X terminals support that). Try to use the command line option "-drvopt defvis", as in ./x01c -dev xwin -drvopt defvis If this works, please report back, we are trying to see if a pattern develops.
