#60 New approval functionality for users module

This patch adds a new link to the "Hello Username"
window that appears after you login in addition to
"Home", "Control Panel" and "Logout". The line is
"Approvals Pending".

What this does is if you are logged in, and you have
authority to approve items, if there exist items to
approve, a link appears in the box allowing you to go
right to the approval module and approve the items that
need approving.

There is a new setting called "show pending approvals"
in the user module that you can check to show the link,
or you can click off to not show the link.

This diff is against 0.10.1 so don't delay :))
