Greetings,

Perhaps you have problem in your operating system, be noted that the 
phpmylibrary runs on red hat linux, using apache, also in windows.

From other questions asked recently, they installed it in slackware, you 
should use at least red hat linux. it will not run on slackware.

be noted also of your installation, you should create a PhpMyLibrary 
directory on your modules directory. i.e. 
postnuke.com/modules/PhpMyLibrary/,.. then put the extracted file index.php 
and the folder polerio, on your modules/PhpMyLibrary directory. Also be 
noted to copy the phpmylibrary.php from polerio/blocks to 
postnuke/includes/blocks directory
