Did you try clearing your browser cache and reloading the page?

Just as a point of note, PMA shouldn't require users to clear out their browser cache every single time it updates. You should make sure the CSS/images are refreshed - this can be accomplished easily by appending the version number to the URL, for example ./themes/pmahomme/img/sprites.png?v=4.0.10

Having said that, for me the problem is usually fixed by logging out then logging back in again.

I'd say that it's not a caching problem in this case - otherwise I'd not have opened an issue.

Is there any way to reproduce this bug ?

try running phpMyAdmin from your git repository, then switch the repository to some other branch and reload the page without clearing your browser cache.

It doesn't seem to happen with my branches. I switched between master and QA_4_1. Nothing went wrong

Which browser do you use please?
Is the icon issue on 1 specific icon or on all icons?
