modified the Death Date and Place report to:
1. Filter by the NAME in addition to the PLAC
2. Include a source column.
3. Made the fonts different and moved the paging/date and time into the header.
One of the problems I am having with the report is if an event has more than one source, only the 1st SOUR is being picked up. However it is working for the most part.

Your changes have been incorporated into the standard reports for Birth, Marriage, and Death. They have also been incorporated in a somewhat different way into the Birth, Death, and Marriage report.
The modifications to these reports will be part of version 4.1.4, which is due to be released within a week or two.
I don't think it's really necessary to print all sources for an event, as long as the first (primary) source is shown.
