This in one of many bugs in v4. You must update your installation for compliance and support as there is no way to remedy your issue with software that is 3.5 years old.

Within this program, header FILES, download the 4.2.3 version, or navigate to CODE > SVN Browse and download the latest Tarball. You can get adequate installation instructions (step-by-step) for upgrading or fresh installation from the wiki: http://wiki.phpgedview.net, as is always the case. Be sure to check system requirements before updating to be sure your server/ISP offers the necessary extensions or you'll have to use 4.2. Also, always the obligatory reminder - BACKUP EVERYTHING, then Back Up again.

Updating PGV should take about 1/2 hour unless your gedcom is quite large (better than 25mb) and then the only delay would be in the reimoprt of the gedcom.
The first line defines your error quite clearly
Undefined index: gedcom_adm_head
I've never heard of an Index folder being defined with such a name. You have made an error in your configuration.
Run /yoursite/install.php again and check your file location specifications.

To be honest, particularly with a move from 4.0 to 4.2+, I would never do an upgrade, but rather a new install, preserving my old site.
Its so easy to create a new DB, or to use a different prefix on the old one, install the software with one admin user, configure the gedcom to my preferences and then move/copy the media (I use media-firewall anyway, so its just a matter of pointing) and any language extra files from the original site's files - I prefer to have a new, complete set of files. Upgrades are OK if you have limited resources - an ISP strict on DB's, little HD space, etc, but this is not the usual situation and the files don't take much space.
Therefore, I'd give a fresh download a try, using at least new prefixes on the DB (42?), and get it running with a fresh import of the gedcom. This should be easy-peasy - I did one yesterday for a friend in about 15 minutes, including the gedcom import.

I just started a new clean installation. Before that, when I went to mydomain.com/phpgedview I got the sign in screen. After inputting my credentials I got 401 error "Authorization Required" - the usual no permission on this server. We will see what happenes next. I had no problems on other sites installing PGV.

OK I solved the 401 issue - now the media files do not show up. Gedcom.ged has clear links to them.
Also there a few boxes I need to check off to import the gedcom.ged from previous version - I am not sure what they are? Thanks for your help - it has been a trip

Media could be several things, but likely (1) permissions on the media folder, or (2) media enabled in the gedcom config.
perhaps you'll post the whys and resolutions of each of the problems you encountered for others who do the same.

Checked permissions and they seem to be OK . In manage Multi media I have "This media object is not linked to any GEDCOM record."Also there are a few that show up but also are not linked.

I did exactly this on export, but I did keep media links on inport. It was sort of intuvitive without any guidence. Now I spent 5 hours deleting all the links and media files. It would help if there was a page note to that fact.

Sorry, but you could have simply reimported and said NO, and you would not have extra links to delete. The help next to the import option was rewritten before the release of 4.2.3 so that it more clearly instructs on these particulars, providing detailed instryctions. Also, there are a number of threads here that advise that, if reimporting a PGV-created gedcom with media links, to select NO. Also, there is a utility in the ADMIN section for deleting duplicate links and finally, the topic is covered in the Upgrading section on the PGV wiki. Finally, you've received prompt, courteously and copious suggestions on this forum and could have asked here.

If you're still having Media trouble, you should try re-importing the original GEDCOM created by your PGV version 4.0.2, this time telling PGV to replace existing information in the database and to NOT keep media links. If the old 4.0.2 installation is stll available and running, you should navigate to the Manage GEDCOMs page and export the database to a new GEDCOM which you can then use for the re-import into the new version of PGV. This will ensure that the media information is correct in the GEDCOM being re-imported. There were problems with older versions of PGV where an existing GEDCOM might not have complete media information if you didn't use the Export option to create a new GEDCOM.
The "Keep media links" option is VERY confusing, even to people who have used PGV for years. The question refers to media links that already exist in the database prior to the re-import.
You would select the "No" option if your incoming GEDCOM contains the right media information and proper links of that media stuff to the various Person, Family, or Source records in the GEDCOM. GEDCOMs created by PGV when you've exercised the "Export" option on the Manage GEDCOMs page will contain the correct media information.
You'd choose "Yes" if the incoming GEDCOM doesn't have any media information in it at all. GEDCOMs created by Family Tree Maker, for instance, don't have any media information. This is useful to people like me, who maintain the genealogical database off-line with something like Family Tree Maker, and then periodically export that FTM database to PhpGedView for presentation on the web.
For the "Yes" option to work properly, the various ID numbers in the GEDCOM would have to be the same from one version of that file to the next.

I appreciate all your help. The whole thing has been a nightmare. I had no problems on two other PGV sites that I run.
So what I did now I cleaned up the gedcom.ged with left over critical errors, destroyed the old site, but kept the media files on my machine. I just started uploading the media files one by one, but there is a problem that the media files do not display. They show properly in the gedcom record and they are being saved on the server, including the thumbs. Bot no picture. What is going on now?

When I put Smith in the Search box I get a whole bunch of various entries of the type
(unknown), Smith + Smith, Joe
Then I noticed that gedcom file has two entries for the female such as
2 _MARNM Smith
2 _MARNM Jane /Smith/
So I deleted one of the _MARNM tags (tried with both tags) and now I get 2 Jane Smiths, with Identical I and two families with identical F
I checked the entries in MySql and found everything to be all right.
However, when I just do a family list search issue does not appear.
It has been driving me crazy for the last few days. I am still using 4.0. 2 version

By the way index is defined as index and located $INDEX_DIRECTORY = "/home/admin5/index/"; The config points to that location.

Do you see the PGV Welcome screen like usual, then login to PGV, then get the authorization message?

401 indicates that you have enabled HTTP authentication on your server. This is nothing to do with PGV.

Hopefully, upon export from your old site, you did select to keep all links and upon import, you did NOT keep media links.
