I set up PHPBTTracker today, and it seems to be exactly what I need. Setup seems good, I can see the index.php file that I set up, and I can log in to the admin interface. I set up the announce URL as http://myserverurl/tracker.php/announce .
Then I created the torrent in uTorrent, marked the box to start seeding, and included the tracker URL as the one listed above. Then I uploaded the torrent that I had created to the tracker as a hidden torrent, and tried to download the file from my neighbors house, but it didn't work. I also tried creating a torrent from my neighbor's house using Tomato, and I got the same result.
The other user never shows up on the list of peers, and no downloading occurs, even though the seeder is seeding the entire file. The tracker seems to see both the seeder and the leacher just fine. What is going on?

It's quite possible that the tracker is giving the leecher the IP but the leecher's firewall is preventing him from making a connection to the seeder.
