This is typically due to a file or directory permission issue.
Try going to Manage->Options->Backup and specifying a directory to which you have write access to as the Backuo Directory at the bottom of the dialog box.

I believe I found the source of this problem. Please look at bug report #1063 that I just posted. From the looks of it, if you do not include the absolute path to your psafe3 file, Pwsafe doesn't know where your file is located because it isn't considering the directory in which the psafe3 is located. For more details look at the bug report.
I checked the permissions, as suggested above, and I have full permissions within the folder the pwsafe files exists, yet I was still having the problem myself. So permissions, in my case, was not the source of the problem.
