[OPT-general] Cannot Upload Documents

Hi,
I am trying to upload documents and every time I get
this:

Document not added -- you must specify a document
file!

I have tried many different files with different
sizes. I can however upload URLs just fine. I have
also tried tweeking the php.ini file to increase the
MAX sizes etc.

Any help will be greatly appreciated.
