When printing a Consult request with the consult_fax_enabled set to true, MRP and Referring Practitioner OHIP No. are not included on the Consultation print page.

NOTE: There was a fix for the bug [#3469] “With consultation_fax_enabled=true, the billing_no is printed on consult but should be ohip_no” in both Master and Release 12_1.

Revert consult request printing to the old process

• consult attachments printed alongside with the consult note
• only print image and pdf attachments
• Lost any NEW requirements regarding printout (e.g. Red urgent status)

Re-tested both Master and Release 15_beta -> it reverts back to the old ui Consult Request printing. Leave the ticket pending for now.

Fax button and Print Preview should also be reviewed for the disabled faxing settings.
